Clark Physician Group offices relocating to Clark Memorial Hospital campus

Includes Cardiology Center Neurology of Southern Indiana and Sleep Center of Southern Indiana

JEFFERSONVILLE, Ind. (Feb. 22, 2016) — The Clark Physician Group’s Cardiology Center Neurology of Southern Indiana and Sleep Center of Southern Indiana are relocating to the Medical Arts building on the Clark Memorial Hospital campus.

Effective March 7, the new address for both practices will be the Medical Arts Building, 207 Sparks Ave., Suite 300, Jeffersonville, Ind. 47130.

For the Cardiology Center Neurology of Southern Indiana Dr. Zaka Rahman, Dr. Juan Ortiz and nurse practitioner Beth Walker will continue to provide high-quality heart care to patients in Clark, Floyd and surrounding counties.

For the Sleep Center of Southern Indiana Dr. Mohammed Hasnain and staff will continue to provide high-quality neurology care to patients in Clark, Floyd and surrounding counties.
