Crisis Phase of Low Income Home Energy Assistance Program begins Jan. 9

New automated appointment line now open to schedule appointments in advance

LOUISVILLE, Ky. (Dec. 29, 2016) — The annual program that helps prevent low income residents of Louisville from being disconnected from utility services begins Monday, Jan. 9, 2017.

LIHEAP, the Low Income Home Energy Assistance Program, is administered by Louisville Metro Department of Community Services. The Crisis Phase of LIHEAP will run from Jan. 9 – March 31, or until available funds have been expended, whichever comes first.

Residents wishing to apply must make an appointment in advance utilizing the new, automated appointment system. The toll free service is now available 24 hours a day, seven days a week.

The caller can follow the prompts to get the information needed and/or to schedule an appointment if available. The system provides a confirmation number indicating that an appointment was successfully scheduled.

• Choose the desired location for your appointment (the six office locations are listed below.)
• Choose a date/time for your appointment
• Record your name – say and spell your first and last names slowly and clearly
• Enter a contact phone number
• Enter your 12-digit LGE account number (if applicable)
• Enter the last four digits of your Social Security number and date of birth
• Receive your appointment confirmation number (have pen/paper ready)
• Appointment details (location, date and time) will be repeated
• List of documents to bring to the appointment will be given

Applicants must be Jefferson County residents with a household income at or below 130 percent of the federal poverty guidelines (e.g. for a household of four, the gross monthly income cannot exceed $2,633.00).

For the Crisis Component, clients must meet the criteria listed above and be in a crisis situation involving imminent loss of energy including one of the following:

• Must have received a brown bill, disconnection notice from their utility service provider, or
• Must have an eviction notice if their utilities are included in the rent, or
• Have four or fewer days’ worth of fuel oil, propane, kerosene, wood or coal available, or
• Must be within ten days or less of running out of prepaid electric service.

All LIHEAP offices will be closed on Mon., Jan. 2 and Mon. Jan. 16, 2017 in observance of the New Year’s Day holiday and Martin Luther King, Jr. Day.

The following documents must be presented at the time of the LIHEAP appointment:

• Disconnect Notice (brown bill from LG&E), or eviction notice if heating expenses are included in the rent. If heat is included in rent, clients must also provide a copy of the lease in addition to the eviction notice.
• Proof of all household income for the preceding month (Food Stamp award letter, Social Security Award letter, pay stubs, etc. or proof of $0 income. Note: Zero Income forms are available at the Community Services office at 701 W. Ormsby Ave., Suite 201 or by clicking here (PDF).)
• Social Security card(s) or official documentation with Social Security numbers or Permanent Resident Card (Green Card) for everyone living in the household.
• Please bring the account number and name on the account for main heating fuel sources and electric.
Benefits provided by this program are paid directly to the applicant’s heating vendor.

Individuals who are unable to apply for this program in person may send a representative on their behalf with a note including the applicant’s signature and phone number. Residents who are homebound and who are unable to send a representative may call 502-780-7937.

LIHEAP applications will be completed at the time of the appointment at one of the six locations listed below. Please note that 810 Barret Avenue is closed and is not a location this year.
